- Use of the Core Content Classification in General Practice (3GCP) for qualitative analysis of context and practice. Ten-year study of undergraduate students’ final works in the Integrated Master's Degree in Medicine at the University of CoimbraPublication . Objectives: To document the domains addressed in the final assignments of the Integrated Master’s Degree in Medicine at the Faculty of Medicine, University of Coimbra, in the area of GP/FM. Material and methods: Observational study of the titles of final assignments, between 2008 and 2017, granted by the Faculty of Medicine of the University of Coimbra. A domain analysis using as codes the International Classification in Primary Care-2 and the Q-Codes, a context classification in Primary Care, year of elaboration and gender of author was carried out for each title of final assignment. A descriptive and inferential analysis was performed through parametric and nonparametric tests. Results: 169 papers were analysed, 23.1% written by male students, with a positive overall growth dynamics (Δ = +7) between 2008 and 2017. Q-Codes were registered 276 times, while the ICPC-2 codes were used 133 times. Under the Q-Codes, “doctor’s issues” is the most frequently addressed (n = 112; 67.2%), and under the International Classification in Primary Care-2 classifications, the chapter “Psychological” was predominant (n = 35; 21%). Under the Q-Codes, subcategories “primary care setting” (n = 26; 15.6%), “health issue management” (n = 23; 13.8%) and “unable to code, unclear” (22; 13.2%) were dominant. Within the International Classification in Primary Care-2, the subcategories “diabetes noninsulin dependent” (n = 22; 13.2%), “depressive disorder” (8, 4.8%) and “hypertension uncomplicated” (8; 4.8%) were predominantly focused on. Conclusions: The 3CGP may become a professional tool, allowing for more precise identification of final works, for a better communication method in medical activity and for avoiding the loss of previously developed works.
- Communication and health: doctor-patient relationship in patients with multimorbidity, an exploratory studyPublication . With the increase of average life expectancy and the aging of the population, an increase in multimorbidity has also been observed. Effective communication in the doctor–patient relationship is directly related to the promotion of health of patents with multimorbidity. One of the fundamental foundations of this doctor-patient relationship is the physician’s ability to establish an empathic relationship with the patent. Objectives. The objective of this study is to compare Family Medicine consultations and Hospital consultations in terms of empathic communication and the doctor-patient relationship in patents with multimorbidity. Material and methods. An exploratory observational cross-sectional study was carried out using elderly people with multimorbidity and applying the JSPE scale. Statistical analysis was performed using descriptive statistics and the non-parametric Mann–Whitney u test. Results. For all questions on the JSPE scale, the family physician scored higher than the hospital physician. Between the specialties, statistically significant differences were obtained. Conclusions. The differences between Family Medicine consultations and Hospital consultations has an impact on the results of the medical empathy felt by the patents. Other studies demonstrate the importance of empathy in the doctor-patient relationship, and in this study, we concluded that there is a greater degree of empathy felt by the patients in Family Medicine consultations compared to the lower degree of empathy felt by the patents in Hospital consultations.
- Impacto Educacional, pelo Método de Ensino de Medicina Geral e Familiar, nos Estudantes de Medicina da Universidade de Coimbra no Ano Letivo 2017-18Publication . Freitas, Mélanie; Santiago, Luiz Miguel de Mendonça Soares; José, Inês; Simões, José Augusto RodriguesObjetivos: Caracterizar o impacto educacional do processo de ensino da unidade curricular (UC) de Medicina Geral e Familiar (MGF), avaliando os resultados do ensino e aprendizagem na UC de MGF no ano letivo de 2017-2018. Métodos: Foram analisados os resultados das respostas a questionário aplicado em sala no início e no fim de cada aula a todos os alunos respondentes. Realizou-se estatística descritiva sendo verificadas as dinâmicas de crescimento das respostas (Δ= (t2-t1)/t1). Adotou-se para significado estatístico o valor de p<0,01. Resultados: Neste estudo, 78,3% das 60 perguntas apresentaram uma dinâmica de crescimento positiva e 21,6% das questões, apresentaram uma dinâmica de crescimento negativa. Houve um ganho em 50% do total de perguntas na melhor resposta no final da aula. Em 50% das questões, não houve avaliação satisfatória quanto à resposta dada 56,7% das perguntas apresentaram resultados estatisticamente significativos. Em 51,7% das perguntas há diferença com significado estatístico entre ambos os tempos de aplicação com p<0,01. Conclusões: Verificou-se ganho na melhoria das respostas no final da aula para os alunos que frequentaram as aulas teórico-práticas. A metodologia aplicada deverá ter futuras re-edições para confirmar os presentes resultados.
