Abstract(s)
Este trabalho encontra-se dividido em duas partes distintas: a primeira que corresponde ao trabalho de investigação desenvolvido na área da Hipertensão Pulmonar e a segunda parte que corresponde ao relatório de estágio realizado na farmácia comunitária.
Capitulo 1: A Hipertensão Pulmonar é definida como uma pressão arterial pulmonar média = 25 mmHg em descanso, obtida por cateterismo cardíaco direito. O termo hipertensão pulmonar arterial descreve um subtipo de hipertensão pulmonar que se caracteriza pela presença de HP pré-capilar. O principal e mais debilitante sintoma desta doença é a dispneia, que vai condicionar a execução das atividades diárias como caminhar. O objetivo deste trabalho passa por analisar a eficácia de dois fármacos (bosentano e sildenafil) no aumento da capacidade física dos doentes com Hipertensão Pulmonar Arterial, nomeadamente em quanto aumenta a distância percorrida no 6MWT (“6-Minute Walk Test”). Foram realizadas duas meta-análises com os artigos recolhidos (total de 10): uma de efeitos fixos e outra de efeitos aleatórios. Na meta-análise de efeitos aleatórios verificou-se que ambos os fármacos aumentaram a distância percorrida (48,4 para o bosentano e 76,3 para o sildenafil). Como a heterogeneidade se apresentava elevada na meta análise de efeitos fixos fez-se uma meta-regressão considerando a idade média, a duração de tratamento e o tipo de estudo. Daqui conclui-se que a diferença média pós-pré-tratamento com o sildenafil tende a ser maior em cerca de 22 metros comparando com o bosentano nas mesmas condições. Desta forma podemos concluir que o sildenafil se mostrou mais eficaz no aumento da distância percorrida no 6MWT.
Capitulo 2: O relatório de estágio em farmácia comunitária resume todo o trabalho desenvolvido ao longo dos seis meses de estágio na Farmácia da Misericórdia. Para além da parte sobre o trabalho desenvolvido aborda também os aspetos legais e técnicos inerentes ao funcionamento técnico e legal de uma farmácia comunitária.
This report is made up of two sections: the first part describes the research analysis about Pulmonary Hypertension and the second one states the work in a Community Pharmacy. Chapter 1: The pulmonary hypertension is defined as a mean pulmonary artery pressure at rest of = 25 mmHg, measured by a right heart catheterization (RHC). Pulmonary arterial hypertension describes a subgroup of pulmonary hypertension characterized by the presence of pre-capillary pulmonary hypertension. The most common and debilitating condition is dyspnea that interferes with the normal daily activities, such as walking. This work was intended to analyse the effectiveness of two drugs (sildenafil and bosentan) in the physical capacity increase of patients with pulmonary arterial hypertension, particularly in increasing the distance covered in the test. In this way, two meta-analyzes were conducted with the collected items (total of 10): fixed effects and other random effects. In the randomized effects meta-analysis of, it has been found that both drugs increased the distance traveled (48.5 m for bosentan and 76.3 for sildenafil). As the highest heterogeneity was presented in the fixed effects analysis, the target became a meta- regression considering the average age, duration of treatment and the type of study. It can be concluded that the average different between post -and pre-treatment with sildenafil tends to be higher (around 22 meters) when compared with bosentan in the same conditions.Thus, we can conclude that sildenafil was more effective in increasing distance in 6MWD. Chapter 2: The report in the Community Pharmacy summarizes all the work performed over the six months traineeship at Farmácia da Misericórdia, Montemor-o-Novo. This document also addresses the legal and technical aspects inherent to the work environment in a Community Pharmacy.
