Abstract(s)
A presente dissertação encontra-se dividida em dois capítulos, sendo o primeiro relativo ao
estágio curricular em farmácia comunitária e o segundo referente à componente de
investigação.
O primeiro capítulo aborda aquilo que foi desenvolvido no estágio curricular, assim como o
papel do Farmacêutico e da Farmácia Comunitária para a sociedade. Pretende-se com este
relatório descrever o funcionamento e o enquadramento da Farmácia Comunitária, bem como
as tarefas e responsabilidades dos seus colaboradores, com especial enfoque no
Farmacêutico.
O segundo capítulo refere-se à: “Adequação do Método Dáder no seguimento
farmacoterapêutico em doentes hipertensos institucionalizados num Lar de Idosos.” O
seguimento farmacoterapêutico é a prática profissional que visa o acompanhamento do
doente e da sua medicação. Quanto maior o risco de problemas relacionados com
medicamentos, mais importante é a realização do seguimento. O Método Dáder já
demonstrou ser um meio eficaz para um seguimento farmacoterapêutico de qualidade.
O crescente envelhecimento demográfico é um dos maiores desafios deste século. Verifica-se
o aumento compreensível de patologias crónicas, como é o caso da hipertensão arterial, e de
medicação associada, bem como a necessidade de acompanhamento destas mudanças
demográficas. Cada vez mais idosos residem em lares, sendo a sua medicação preparada e
administrada por terceiros, muitas vezes sem formação na área da saúde.
Esta investigação teve como objectivo estudar a adequação do Método Dáder de seguimento
farmacoterapêutico associado a um lar de idosos. O método revelou-se eficaz na detecção de
problemas relacionados com medicamentos, deixando uma porta aberta a novos estudos que
apliquem o seguimento farmacoterapêutico nestas instituições, a bem de um uso mais seguro
e mais eficaz dos medicamentos.
The present dissertation is divided into two chapters, the first concerning the traineeship in Community Pharmacy and the second related to the research component. The first chapter discusses what has been developed in traineeship, as well as the role of Community Pharmacist and Pharmacy to the society. The intention of this report is to describe the environment and way of functioning of Community Pharmacy, as well as the tasks and responsibilities of its employees, with special focus on the Pharmacist. The second chapter refers to: "Appropriateness of the Dáder Method of pharmacotherapy monitoring in hypertensive patients institutionalized in a geriatric home." Pharmacotherapy monitoring is the professional practice which aims the monitorization of the patient and his medication. The higher the risk of drug-related problems, the more important is the execution of monitorization. The Dáder method has already proven to be an effective way towards a good pharmacotherapy monitoring. The increased demographic aging is one of the biggest challenges of this century. There is an understandable increase of chronic diseases such as hypertension and associated medication, as well as the need to follow these demographic changes. More and more elderly people reside in care homes, with their medications being prepared and administered by others, often with no education in healthcare area. This research aimed to study the appropriateness of the Dáder method of pharmacotherapeutic monitoring associated with a nursing home. The method proved to be effective in detecting drug-related problems, leaving an open door to further studies to apply pharmacotherapeutic monitoring in these institutions, to the safer and more effective use of drugs.
