A presente investigação teve como objetivo analisar de que forma a aplicação da filosofia Lean Healthcare pode contribuir para a eficiência operacional, a segurança e a gestão de custos associados aos instrumentais cirúrgicos no Bloco Operatório Central da Unidade Local de Saúde da Guarda. Assente na metodologia de Investigação-Ação, o estudo promoveu a participação ativa da equipa de enfermagem na identificação de desperdícios, planeamento e implementação de melhorias. Através da aplicação de ferramentas Lean — nomeadamente o Value Stream Mapping (VSM), o 5S, a padronização de processos e os ciclos Kaizen — foi possível identificar e eliminar etapas desnecessárias, reduzir o tempo de preparação das caixas de instrumental cirúrgico e otimizar a organização das arrecadações. Os resultados evidenciaram ganhos significativos na eficiência do serviço e na segurança do doente, reforçando o impacto positivo da filosofia Lean na racionalização dos fluxos e na eliminação de desperdícios. Para além dos benefícios operacionais, a investigação fomentou uma cultura de melhoria contínua, baseada no envolvimento multidisciplinar e na comunicação entre profissionais, consolidando o Lean Healthcare como uma filosofia promotora de qualidade, segurança e sustentabilidade organizacional. Apesar de limitações associadas ao curto período de implementação e ao caráter de estudo de caso, os resultados obtidos apresentam implicações práticas relevantes para a gestão hospitalar. Recomenda-se a continuidade e expansão do projeto a outros contextos e especialidades, bem como o acompanhamento longitudinal dos indicadores de desempenho.
This study aimed to analyze how the application of Lean Healthcare principles can contribute to operational efficiency, safety, and cost management related to surgical instruments in the Central Operating Room of the Local Health Unit of Guarda. Based on an Action Research methodology, the study encouraged the active participation of the nursing team in identifying waste, planning, and implementing improvement actions. Through the application of Lean tools — namely Value Stream Mapping (VSM), 5S, process standardization, and Kaizen cycles — it was possible to identify and eliminate non-valueadded activities, reduce the preparation time of surgical instrument sets, and optimize the organization of storage areas. The results showed significant gains in service efficiency and patient safety, reinforcing the positive impact of the Lean philosophy in streamlining workflows and eliminating waste. Beyond operational benefits, the research fostered a culture of continuous improvement based on multidisciplinary involvement and effective communication among professionals, consolidating Lean Healthcare as a philosophy that promotes quality, safety, and organizational sustainability. Despite limitations associated with the short implementation period and the case study nature, the results present relevant practical implications for hospital management. It is recommended that the project be extended to other surgical specialties and contexts, as well as that the performance indicators be monitored longitudinally.
