Este relatório encontra-se dividido em trĆŖs capĆtulos. O primeiro e o segundo abordam as experiĆŖncias profissionalizantes nas vertentes de FarmĆ”cia Hospitalar e ComunitĆ”ria, respetivamente. Por fim, o terceiro capĆtulo Ć© referente Ć vertente de investigação abordando o tema Polimedicação em Idosos Institucionalizados.
O estĆ”gio em FarmĆ”cia Hospitalar (CapĆtulo I) decorreu, entre 26 de janeiro e 21 marƧo, nos ServiƧos FarmacĆŖuticos do Centro Hospitalar Vila Nova Gaia/Espinho. Durante este perĆodo foi possĆvel, nĆ£o só constatar a importĆ¢ncia do farmacĆŖutico hospitalar enquanto promotor do uso racional e seguro do medicamento, como tambĆ©m contactar com a realidade dos ensaios clĆnicos em meio hospitalar.
O estĆ”gio em FarmĆ”cia ComunitĆ”ria (CapĆtulo II) decorreu, no perĆodo de 23 marƧo a 13 de junho, na FarmĆ”cia da Moderna, em Figueira de Castelo Rodrigo e constituiu uma ótima preparação para os desafios futuros esperados. Com este relatório pretende-se descrever toda a organização e atividades desenvolvidas durante o estĆ”gio e sua articulação com a legislação em vigor.
O CapĆtulo III trata da problemĆ”tica da polimedicação em idosos institucionalizados. A polifarmĆ”cia Ć© um fator relevante nos idosos, uma vez que aumenta o risco de duplicação de medicação, interaƧƵes medicamentosas e reaƧƵes adversas a medicamentos. Os idosos institucionalizados apresentam riscos aumentados, por apresentarem muitas doenƧas limitantes, fragilidade e baixa funcionalidade. Neste estudo apurou-se que 61% da população institucionalizada apresentava polimedicação (definida como o uso de mais de 5 medicamentos em simultĆ¢neo). A faixa etĆ”ria dos 85-94 anos foi a que apresentou maior percentagem (com 68%). Relativamente ao gĆ©nero, o sexo masculino foi o que revelou mais casos com 66% da população masculina. De entre os medicamentos mais prescritos destacaram-se os fĆ”rmacos com ação no aparelho cardiovascular, sistema nervoso central e sangue com 35%, 29% e 12%, respetivamente.
This document is divided into 3 chapters. The first and the second chapters present the work developed in two different professional internships, while the third one presents the research conducted about the Polymedication on Institutionalized Elders in Figueira de Castelo Rodrigo area. Chapter I presents a report on the internship carried between January 26th 2015 and March 21st of the same year, at the Pharmaceutical Services department in the Centro Hospitalar Vila Nova de Gaia/Espinho. During the period of the internship, it was made clear the importance of the Hospital Pharmacists as promoters of the rational and safe use of medication. Additionally it was possible to have contact with clinical trials carried out by the same hospital. Chapter II is a reports on the internship on Community Pharmacy that took place between March 23rd and June 13th of 2015, at āFarmĆ”cia Modernaā in Figueira de Castelo Rodrigo. The report intends to describe all the activities carried out during the period of internship as well as describe the legislation of the sector. This internship was crucial to anticipate and better prepare the challenges that are still to come. Chapter III deals with the polymedication problem present among institutionalized elderly. The polypharmacy is a significant problem, since it increases the likelihood of medication duplication, drug interactions and adverse drug reactions. The institutionalized elderly have an increased risk because they present many limiting diseases, fragility and low functionality. In this study it was found 61% of the studied population had polypharmacy (defined as consuming more than five drugs simultaneously). The age group with the highest percentage of polymedicated was of 85-94 years with 68% of Polymedicated. Additionally, males were the gender most prone to polymedication with 66% of male individuals. Among the most prescribed medications stood out the agents acting on the cardiovascular system, central nervous system and blood with 35%, 29% and 12% respectively.
