A abordagem da via aérea é uma das competências centrais da atividade dos médicos
anestesiologistas, sendo também praticada por médicos de outras especialidades.
A Sociedade Portuguesa de Anestesiologia define via aérea difícil como uma situação
clínica em que um anestesiologista treinado e experiente se depara com dificuldades na
ventilação por máscara facial ou dispositivo supraglótico, dificuldade na laringoscopia
direta ou indireta, dificuldade na intubação traqueal ou na via aérea cirúrgica.
As situações de via aérea difícil constituem-se como as principais causas de morbilidade
e mortalidade relativas a procedimentos anestésicos. Assim, é essencial que se antecipem
as potenciais dificuldades e que se adotem e treinem algoritmos de atuação face a
situaçoes de via aérea difícil previsível e não previsível.
Sempre que um anestesiologista deteta uma via aérea difícil, é fundamental que fique
registado no processo clínico do doente e que este seja portador dessa informação. Esta
medida é essencial, para que, em abordagens futuras, o profissional se prepare
adequadamente e, assim, reduza a morbimortalidade do doente.
Difficult airway management is an area of extreme importance for anesthesiologists, being also practiced by medical doctors of other areas. Portuguese Society of Anesthesiology defines a difficult airway as a clinical situation in which a trained and experienced anesthesiologist is faced with difficulties in bag mask ventilation or ventilation with supraglottic device, difficulty in direct or indirect laryngoscopy, difficulty in tracheal intubation or in surgical airway. Incidents related to difficult airway constitutes the main causes of morbidity and mortality related to anesthetic procedures. This way, it´s essential to anticipate potential problems and adopt and train atuation algorithms to face situations of predictable or unpredictable difficult airway. Anytime an anesthesiologist diagnose a difficult airway situation, this information must be registered in the clinical process and the patient has be informed. This information is essential for future situations, to the professional get prepared in advance, this way reducing patient morbimortality.
