A implementação de compras públicas sustentáveis no sector de saúde constitui uma
estratégia essencial para a otimização dos recursos públicos, ao mesmo tempo em que
promove uma gestão mais eficiente, transparente e comprometida com os princípios do
desenvolvimento sustentável. Nesse contexto, observa-se um esforço normativo
relevante, especialmente com a promulgação da Lei nº 14.133/2021, que estabelece
diretrizes modernas para as aquisições públicas no Brasil. No entanto, apesar desses
avanços legislativos, a efetivação de práticas sustentáveis na gestão das compras
públicas em saúde ainda enfrenta obstáculos significativos relacionados a aspectos
estruturais, culturais e operacionais do sistema público brasileiro.
Esta revisão sistemática tem por finalidade examinar a produção científica disponível
acerca das compras públicas sustentáveis no contexto da saúde no Brasil, com o intuito
de mapear os benefícios evidenciados e os principais desafios enfrentados. A
investigação busca reunir subsídios que permitam compreender os resultados das
políticas adotadas, bem como os fatores que limitam a incorporação efetiva de
princípios de sustentabilidade nos procedimentos de aquisição pública.
Foi realizada uma revisão sistemática da literatura baseada nas Guidelines PRISMA
2020, além de utilizar as bases de dados Scopus, Google Scholar e CAPES, com
orientação nos critérios de inclusão previamente definidos. Como resultados, foram
identificados 2.630 estudos científicos, nas três bases de dados distintas. Após a
aplicação dos critérios de inclusão e exclusão, 29 estudos foram selecionados. Todos os
artigos selecionados passaram por uma avaliação rigorosa, considerando os critérios de
qualidade metodológica e relevância para o tema em questão. Os estudos selecionados
abordam o tema das compras públicas sustentáveis no sector de saúde, com destaque
para artigos provenientes do contexto brasileiro.
Apesar de avanços, como as políticas para fortalecer o SUS, as compras públicas
sustentáveis na saúde ainda enfrentam desafios, como baixa padronização, capacitação
limitada e fragilidade institucional. A prática é fragmentada e distante da legislação,
exigindo uma gestão hospitalar mais sustentável, com governança participativa e uso
estratégico do poder de compra do Estado. Consolidar essa mudança requer formação
contínua, monitoramento eficaz e compromisso ético dos gestores públicos.
The implementation of sustainable public procurement in the healthcare sector represents a key strategy for optimizing public resources, while also fostering more efficient, transparent management aligned with the principles of sustainable development. In this context, a notable regulatory effort can be observed, particularly with the enactment of Law No. 14,133/2021, which establishes modern guidelines for public procurement in Brazil. However, despite these legislative advancements, the effective adoption of sustainable practices in healthcare procurement still faces significant challenges, stemming from structural, cultural, and operational aspects of the Brazilian public system. This systematic review aims to examine the available scientific literature on sustainable public procurement in the context of healthcare in Brazil, with the goal of mapping the reported benefits and the main challenges encountered. The study seeks to gather insights that help understand the outcomes of the policies adopted, as well as the factors that hinder the effective integration of sustainability principles into public procurement processes. A systematic literature review was conducted based on the PRISMA 2020 Guidelines. The databases Scopus, Google Scholar, and CAPES were used, following previously defined inclusion criteria. As results, a total of 2,263 scientific studies were identified across three different databases. After applying the inclusion and exclusion criteria, 29 studies were selected. All selected articles underwent a rigorous evaluation based on methodological quality and relevance to the topic. The selected studies focus on sustainable public procurement in the healthcare sector, with an emphasis on articles from the Brazilian context. Despite advances such as policies aimed at strengthening the SUS, sustainable public procurement in the healthcare sector still faces challenges such as low standardization, limited training, and institutional fragility. The implementation remains fragmented and disconnected from existing legislation, highlighting the need for more sustainable hospital management, with participatory governance and strategic use of the State’s purchasing power. Consolidating this shift requires continuous training, effective monitoring, and the ethical commitment of public managers.
