Introdução: A técnica Wide Awake Local Anesthesia No Tourniquet (WALANT) tem
emergido como uma técnica anestésica inovadora, eficaz e segura. A utilização desta técnica
permite a realização de intervenções cirúrgicas com o utente acordado, apresentando
vantagens para os utentes, cirurgiões e instituições de saúde.
Objetivo principal: Estudar a utilização da técnica WALANT no tratamento cirúrgico da
patologia da mão na ULS Cova da Beira.
Materiais e métodos: Estudo retrospetivo, transversal e descritivo. Através da consulta dos
processos clínicos na plataforma S. Clínico foram analisadas 328 intervenções cirúrgicas
recorrendo à técnica WALANT, durante o período de 22/03/2021 a 21/03/2022 no serviço
de ortopedia da Unidade Local de Saúde da Cova da Beira. A análise estatística foi realizada
recorrendo ao Statistical Package for the Social Sciences (SPSS 29.0.1.0) e utilizando o teste
do Qui-quadrado, o teste exato de Fisher e o teste de Z para proporções.
Resultados: Foram analisadas 328 intervenções cirúrgicas recorrendo à técnica WALANT,
envolvendo 48 patologias da mão e punho. A idade média da população em estudo foi 59
anos, sendo esta constituída por 213 utentes do sexo feminino e 115 do sexo masculino. As
patologias mais frequentes foram a síndrome do túnel cárpico, contratura de Dupuytren e
dedo em gatilho. Não foram registadas complicações diretamente relacionadas com o uso
da técnica WALANT.
Conclusão: A técnica WALANT tem se afirmado como uma abordagem promissora no
tratamento cirúrgico da patologia da mão, alinhada com o princípio value-based dos
sistemas de saúde. No futuro, é expectável a sua crescente utilização em procedimentos inoffice, a realização de estudos prospetivos para melhor caracterizar o impacto do seu uso e
a sua integração em mais instituições, consolidando o seu papel como paradigma na cirurgia
da mão.

The use of this technique allows surgical interventions to be carried out while the patient is awake, with advantages for patients, surgeons and healthcare institutions. Main objective: To study the use of WALANT technique in the surgical treatment of hand pathology at ULS Cova da Beira. Materials and methods: Retrospective, cross-sectional and descriptive study. By consulting the clinical files on the S. Statistical analysis was performed using the Statistical Package for the Social Sciences (SPSS 29.0.1.0) software and using the Chisquare test, Fisher's exact test and the Z-test for proportions. Results: 328 surgical interventions using WALANT technique were analyzed, involving 48 pathologies of the hand and wrist. The average age of the study population was 59, and 213 of the patients were female and 115 male. The most frequent pathologies were carpal tunnel syndrome, Dupuytren's contracture and trigger finger. There were no complications directly related to the use of the WALANT technique. Conclusion: WALANT technique has established itself as a promising approach to the surgical treatment of hand pathology, in line with the value-based vision of healthcare systems. In the future, its growing use in in-office procedures, prospective studies to better characterize the impact of its use and its integration in more institutions are to be expected, consolidating its role as a paradigm in hand surgery.
