A saúde da população é fundamental para a felicidade e o bem-estar de todos os
indivíduos, sendo os medicamentos parte essencial e crítica dos serviços de saúde em todas as
sociedades e culturas. O farmacêutico é o elemento de articulação entre a medicina e a
Humanidade, desempenhando atividades nas diferentes áreas do circuito do medicamento.
No contexto da experiência profissionalizante do Mestrado Integrado em Ciências
Farmacêuticas da Universidade da Beira Interior, o presente trabalho encontra-se dividido em
três componentes, entre as quais: o projeto de investigação de mestrado intitulado “Análise
da Monitorização Farmacocinética da Gentamicina nos Serviços Farmacêuticos do Centro
Hospitalar Cova da Beira (CHCB)”, o relatório de estágio em Farmácia Hospitalar e o relatório
de estágio em Farmácia Comunitária.
No que concerne ao projeto de investigação de mestrado, a monitorização de
concentrações séricas de fármacos é uma das intervenções assistenciais que tem vindo a ser
desenvolvida pelo farmacêutico hospitalar, no âmbito da sua atividade clínica. A consideração
de critérios farmacocinéticos na elaboração de regimes posológicos contribui
significativamente para uma melhoria da eficácia e segurança dos tratamentos
farmacológicos, promovendo uma utilização mais racional do medicamento. Para além do
caráter assistencial, um serviço de farmacocinética clínica deve envolver-se ativamente numa
vertente de investigação e didática, sendo que todos os dados gerados da atividade de
monitorização constituem uma base para análise posterior. Neste sentido, a investigação de
caris retrospetivo que foi levada a cabo pretendeu caraterizar a monitorização
farmacocinética da gentamicina realizada nos Serviços Farmacêuticos do CHCB, durante os
últimos cinco anos. A interceção dos dados resultantes da monitorização com a história clínica
dos doentes permitiu avaliar a influência de diferentes variáveis no perfil farmacocinético e
na uniformização posológica da gentamicina, bem como o grupo de doentes que beneficiam
com esta intervenção. Ao nível da experiência profissional em Farmácia Hospitalar, o relatório de estágio
pretendeu refletir sobre a estrutura e metodologia dos Serviços Farmacêuticos Hospitalares, assim como sobre as competências técnico-científicas do farmacêutico integrado nas equipas
multidisciplinares de saúde. Durante o período de estágio pude contactar com as diferentes
áreas de atuação do farmacêutico hospitalar, entre as quais: a farmacotecnia, a gestão e a
logística dos Serviços Farmacêuticos, os diferentes circuitos de distribuição, a farmácia
clínica, os ensaios clínicos e as comissões de apoio técnico. Esta experiência levou-me a
concluir que o farmacêutico hospitalar, enquanto especialista e gestor da terapêutica
medicamentosa, destaca-se como uma mais-valia em todo o circuito do medicamento
providenciado pelas instituições hospitalares, cumprindo cada vez mais com as exigências que lhe são propostas. Por fim, a Farmácia Comunitária é uma unidade de Saúde onde são prestados serviços
de intervenção farmacêutica com elevada diferenciação técnico-científica e de âmbito
extremamente diversificado, indo ao encontro das necessidades dos doentes e da população.
Esta é, sem dúvida, uma das portas de entrada no Sistema de Saúde, dada a sua
acessibilidade e posição privilegiada face à sociedade. Nesta vertente, também este relatório visou descrever o âmbito profissional do farmacêutico comunitário e as atividades realizadas ao longo do estágio.
The population health is essential for the happiness and well-being of all the individuals, being the medicine the main and critical share of health services in all societies and cultures. The chemist is the element of joint between medicine and Humanity, playing activities in the different areas of the medicine circuit. In the context of professional experience in the Master’s degree integrated in Pharmaceutical Sciences from Beira Interior University, the present work is divided in three components, which are: the master’s degree research project named “Analysis of the Gentamicin Pharmacokinetic Monitoring in the Pharmaceutical Services of the Hospital Cova da Beira (CHCB)”, the traineeship report in Hospital Pharmacy and the traineeship report in Communitarian Pharmacy. With respect to master’s degree research project, the drugs monitoring of serum is one of the care interventions which has been developed by the hospital chemist, in the scope of his clinical activity. The consideration of pharmacokinetic criterions in the development of dosing regiments significantly contributes to improving the effectiveness and safety of pharmacological treatments, promoting a more rational use of medication. Beyond the assisting character, a clinical pharmacokinetic service should be actively involved in a trend of research and teaching, and all data generated from monitoring activity form a foundation for later review. In this sense, the retrospective nature of research that was carried out intended to characterize the gentamicin pharmacokinetic monitoring performed in the Pharmaceutical services in CHCB, during the last five years. The intersection of data resulting from the monitoring with the clinical history of patients allow assessment of the influence of different variables on the pharmacokinetic profile and on the standardization of gentamicin dosage, well as the group of patients who benefit from this intervention. In terms of the professional experience in Hospital Pharmacy, the traineeship report intended to reflect about the structure and methodology of the Hospital Pharmaceutical Services, and also about the technical and scientific skills of the chemist integrated into multidisciplinary health teams. During the traineeship period I could contact with the different areas of the hospital chemist, including: the Pharmacotechnics, management and logistics of Pharmaceutical Services, the different distribution channels, the clinical pharmacy, clinical trials and technical support commissions. This experience led me to conclude that the hospital chemist as a specialist and manager of drug therapy, stand out as an asset to the entire circuit of the drug provided by hospitals, serving more and more with the demands offered to him. Finally, the Communitarian Pharmacy is a health unit where pharmaceutical interventions services are provided with high technical and scientific differentiation and in an extremely diversified context meeting the patients and population needs. This is undoubtedly one doorway into the health system, given its accessibility and privileged position to society. In this aspect, this report aimed to describe the professional scope of a Communitarian chemist and the activities performed during the traineeship.
