O presente relatório encontra-se dividido em dois capítulos. O primeiro capítulo refere-se à componente de estágio em farmácia comunitária, enquanto o segundo capítulo descreve o trabalho de investigação realizado.
O estágio em farmácia comunitária foi realizado na Farmácia Luciano & Matos em Coimbra sob orientação da Dr.ª Maria Helena Amado, perfazendo um total de 800 horas. Neste capítulo é apresentado o espaço físico e equipa constituinte da farmácia LM, e são descritas todas as atividades experienciadas durante os 5 meses de estágio.
O projeto de investigação tem como objetivo avaliar a intervenção do farmacêutico em doentes com diabetes mellitus tipo 2 em contexto de farmácia comunitária. Foram estudados 10 casos de consultas farmacêuticas baseadas no Método Dáder realizadas entre 2011 e 2013. Os dados das consultas foram cedidos por uma farmácia comunitária em Portugal, que por questões de ética e confidencialidade foi designada ao longo do trabalho como “farmácia do estudo”. A maioria dos doentes apresentava vários problemas de saúde descontrolados, foi detetado um caso de duplicação da medicação e três casos foram reportados ao médico resultando num ajuste terapêutico. Com este trabalho foi possível constatar a importância do farmacêutico, mas também identificar algumas dificuldades e melhorias na implementação deste serviço.
The present review is divided in two chapters. The first chapter addresses the stage component in community pharmacy, while the second chapter describes the investigation work. The stage in community pharmacy was realized in the Luciano & Matos pharmacy in Coimbra, with a total of 800 hours. In this chapter the physical space of the pharmacy and the staff team is presented, as well all the activities experienced during the 5 month period. The main goal of the investigation work was to evaluate the pharmacist intervention in patients with diabetes mellitus type 2 in a community pharmacy context. 10 cases of pharmaceutical care realized between 2011 and 2013 based on the Dáder method where studied. The data from the cases where handed by a community pharmacy based in Portugal. Due to ethic and confidentiality reasons this pharmacy was designed within the work as the “study pharmacy”. Most of the patients had several health conditions uncontrolled. In one case the patient was doubling one medication and three other cases where reported to the respective patient doctors, which resulted in a therapeutic change. With this work it was possible to enhance the importance of the pharmacist, but also indentify some difficulties and outcomes in the implementation of the pharmaceutical care.
