A presente Dissertação de Mestrado encontra-se dividida em dois capítulos: o Capítulo 1
aborda uma vertente de Investigação e o Capítulo 2 está relacionado com o estágio
curricular em Farmácia Comunitária.
No primeiro Capítulo, após uma revisão de literatura relativa ao tema de interesse, definiuse
como objetivo do estudo a avaliação da terapêutica anti-inflamatória não esteroide na
sintomatologia e estado dos doentes idosos com demência e Doença de Alzheimer da
EBIcohort (Elderly of Beira Interior Cohort), no contexto do projeto ICON
(Interdisciplinary Challenges On Neurodegeneration; CENTRO-01-0145-FEDER-000013
)Para alcançar o objetivo pretendido com o estudo, propôs-se realizar uma abordagem que
inclui vários aspetos relacionados com os doentes, sendo utilizado o Exame Cognitivo de
Addenbrooke-Revisto (ACE-R), que permitirá determinar se a terapia anti-inflamatória
teve impacto na cognição dos idosos em estudo.
Além disso, foram consideradas as propriedades motoras, como a marcha e a força da mão
dominante, para avaliar como a terapia afeta a função motora desses doentes. Parâmetros
bioquímicos também serão analisados para obter uma compreensão mais completa dos
efeitos da terapêutica. Com o intuito de avaliar as diferenças nas características dos doentes
em análise, este projeto realizará uma comparação com um grupo controlo e com mais três
grupos de estudo com diferentes graus de comprometimento cognitivo. Esta distribuição
pelos grupos de declínio cognitivo foi realizada com o auxílio da Escala de Deterioração
Global (GDS) que avalia o grau de deterioração cognitivo em sete estágios. Ao longo do
estudo estes quatro grupos de estudo foram submetidos a possíveis correlações com
diversas variáveis em questão, entre as quais o sexo, idade, IMC e outras variáveis
supracitadas. As análises propostas permitirão uma abordagem abrangente e integrada,
proporcionando uma compreensão mais aprofundada sobre os potenciais benefícios ou
limitações dessa forma de tratamento específica.
Por fim dentro de cada grupo realizou-se uma distinção dos idosos que tomam e os idosos
que não tomavam anti-inflamatórios não esteroides (AINE) de modo a poder ter uma
perceção de como esta terapêutica se relacionava ao longo dos grupos de estudo. Os
resultados demonstraram várias correlações significativas entre algumas das variáveis e os
grupos de declínio cognitivo. Porém a última análise foi inconclusiva, apesar de se ter
observado pequenas diferenças nos grupos que tomavam AINE ou não tomavam. O segundo capítulo aborda a minha experiência em farmácia comunitária na Farmácia
dos Pastorinhos, com localização na cidade de Fátima, sob orientação do Dr. Jonatas
Mangas, o entre os dias 6 de fevereiro a 21 de junho. Esta experiência permitiu consolidar
e aplicar todos os conceitos teóricos e práticos adquiridos ao longo do curso.
The present Master's Dissertation is divided into two chapters: Chapter 1 addresses a research aspect, while Chapter 2 is related to the curricular internship in Community Pharmacy. In the first chapter, following a literature review on the topic of interest, the study's objective was defined as the assessment of non-steroidal anti-inflammatory therapy in the symptoms and condition of elderly patients with dementia and Alzheimer's disease from the EBIcohort (Elderly of Beira Interior Cohort), within the context of the ICON project (Interdisciplinary Challenges On Neurodegeneration; CENTRO-01-0145-FEDER-000013). To achieve the intended objective, an approach encompassing various aspects related to the patients was proposed. The Revised Addenbrooke Cognitive Examination (ACE-R) was used to determine if anti-inflammatory therapy had an impact on the cognition of the elderly under study. Additionally, motor properties such as gait and dominant hand strength were considered to assess how therapy affects the motor function of these patients. Biochemical parameters were also analyzed to gain a more comprehensive understanding of the effects of the therapy. In order to assess differences in the characteristics of the analyzed patients, this project will compare them with a control group and three more study groups with different degrees of cognitive impairment. The distribution across cognitive decline groups was performed with the help of the Global Deterioration Scale (GDS), which evaluates the degree of cognitive deterioration in seven stages. Throughout the study, these four study groups were subjected to possible correlations with various variables, including gender, age, BMI, and other aforementioned variables. The proposed analyses will allow for a comprehensive and integrated approach, providing a deeper understanding of the potential benefits or limitations of this specific form of treatment. Finally, within each group, a distinction was made between the elderly who took non-steroidal anti-inflammatory drugs (NSAIDs) and those who did not, to perceive how this therapy related across the study groups. The results showed several significant correlations between some variables and cognitive decline groups. However, the final analysis was inconclusive, despite observing small differences in groups taking NSAIDs or not.
