O relatório apresentado, realizado para a obtenção do grau de Mestre em Ciências
Farmacêuticas encontra-se dividido em 3 capítulos. O primeiro capítulo é referente ao
relatório de estágio realizado em Farmácia Comunitária, no segundo capítulo é
apresentado o relatório de estágio em Farmácia Hospitalar e o terceiro e último
capítulo, na vertente da investigação, avalia a gestão de medicação em ERPI’s do
concelho de Idanha-a-Nova.
No primeiro capítulo é apresentado o relatório do estágio realizado na Farmácia
Pereira Rebelo, localizada em Castelo Branco. Este estágio iniciou a 4 de Novembro de
2024 e finalizou a 31 de Janeiro de 2025. Durante este período pude compreender e
assimilar as competências de um farmacêutico comunitário.
Já no segundo capítulo, referente ao estágio decorrido na Unidade Local de Saúde de
Castelo Branco, pude compreender as variadas funções e responsabilidades que recaem
sobre o farmacêutico hospitalar. Este estágio teve início no dia 9 de setembro de 2024 e
finalizou no dia 31 de outubro de 2024.
Por fim, o terceiro capítulo, na vertente de investigação, avalia a forma como 6
Estruturas de Residência para Pessoas Idosas (ERPI) no concelho de Idanha-a-Nova
organizam a sua gestão de medicação. Para isso, foram realizadas visitas a todas as
instituições onde foi possível observar como a gestão é realizada. Além disso foi
aplicado um questionário para avaliar se esta gestão está de acordo com o Manual de
Boas Práticas de Farmácia Hospitalar, uma vez que, tal como no hospital, estas
instituições albergam muitos utentes polimedicados e com necessidades acrescidas de
cuidados de saúde.
The present report, is divided into three chapters. The first chapter refers to the internship report carried out in a Community Pharmacy. The second chapter presents the internship report conducted in a Hospital Pharmacy. The third and final chapter, focused on the research component, evaluates medication management in Residential Structures for the Elderly (ERPI) in the municipality of Idanha-a-Nova. The first chapter presents the internship report carried out at Farmácia Pereira Rebelo, located in Castelo Branco. This internship began on November 4th, 2024, and ended on January 31st, 2025. In this period, I was able to understand and develop the core competencies of a community pharmacist. The second chapter refers to the internship carried out at the Local Health Unit of Castelo Branco, where I was able to understand the various functions and responsibilities assigned to a hospital pharmacist. This internship began on September 9th, 2024 Finally, the third chapter, corresponding to the research component, evaluates how six residential structures for the elderly in Idanha-a-Nova organize their medication management. Visits were carried out to all institutions, allowing direct observation of how medication management is implemented. In addition, a questionnaire was made to assess whether this management complies with the Manual of Good Hospital Pharmacy Practices, given that, as in hospitals, these institutions accommodate many polymedicated patients with increased healthcare needs., and ended on October 31st, 2024.
