Abstract(s)
Introdução: Nos Ășltimos anos os Serviços de UrgĂȘncia tĂȘm-se transformado progressivamente na porta de entrada do Serviço Nacional de SaĂșde e estima-se que 50% dos episĂłdios registados nĂŁo sejam apropriados ao tipo de atendimento prestado nestes Serviços. Isto acarreta consequĂȘncias negativas para o seu funcionamento geral. Os hiperfrequentadores constituem uma importante fatia dos utentes dos Serviços de UrgĂȘncia e requerem, com frequĂȘncia, mais cuidados e recursos fĂsicos do que os restantes utentes.
Metodologia: Estudo retrospectivo com anĂĄlise documental dos processos clĂnicos da UrgĂȘncia dos utentes que recorreram ao Serviço de UrgĂȘncia do Centro Hospitalar da Cova da Beira durante o ano de 2009. Foram considerados hiperutilizadores todos os utentes que recorreram ao Serviço quatro ou mais vezes no ano em estudo. Analisaram-se as variĂĄveis sexo, idade, causa de admissĂŁo, dia e hora de admissĂŁo, triagem, discriminador, tempo de permanĂȘncia e destino.
Resultados: Os hiperutilizadores, que em 2009 constituĂram 12,6% dos utentes, perfizeram 35,8% das visitas ao Serviço de UrgĂȘncia do Centro Hospitalar da Cova da Beira durante. Comparativamente aos restantes utentes, estes eram com mais frequĂȘncia mulheres (58,9% vs. 53,9% p<0,001), tinham idade superior (59 vs. 51 anos p<0,001) e maior nĂvel de urgĂȘncia, conferido por uma das triagens Amarelo, Laranja e Vermelho (55,5% vs. 51,6% p<0,01). Estes utentes permaneceram mais tempo no Serviço do que os restantes (184 vs. 159 minutos p<0,001).
ConclusĂŁo: O hiperutilizador do Serviço de UrgĂȘncia Geral do Centro Hospitalar da Cova da Beira Ă© caracteristicamente mulher, na casa dos 60 anos, que Ă© admitido no SU por âdoençaâ. Recebe triagem Pouco urgente ou Urgente na maioria dos casos, devido a âdorâ ou âdor moderadaâ e permanece no SU 127 minutos. Ă mais velho, tem triagem âmuito urgenteâ ou âemergenteâ com maior frequĂȘncia do que os outros utentes e tempo de permanĂȘncia no Serviço de UrgĂȘncia substancialmente maior do que os restantes utentes.
Introduction: In recent years the Emergency Department has increasingly become the gateway to the National Health Service and it is estimated that 50% of the events recorded are not appropriate for the type of care provided in these Services. This has negative consequences for their overall functioning. The frequent users constitute an important share of users of Emergency Departments and often require more care and physical resources than other users. Methodology: A retrospective study with documentary analysis of the clinical charts of the users who resorted to the emergency room of the Hospital of Cova da Beira during the year 2009. We considered frequent users all the patients that used the service four or more times in the year under study. We analyzed gender, age, cause of admission, date and time of admission, triage, discriminator, duration and destination. Results: Frequent users, which in 2009 constituted 12.6% of users, totaled 35.8% of visits to SUCHCB. Compared to other users, they were more often women (58.9% vs. 53.9% p <0.001), had higher age (59 vs. 51 years p <0.001) and higher level of urgency, given by one of screenings Yellow, Orange and Red (55.5% vs. 51.6% p <0.01). These users spent more time in the department than the others (184 vs. 159 minutes p <0.001). Conclusion: Frequent Emergency Room users are typically women at around 60 years who are admitted for "disease". Receives Non Urgent or Urgent triage in most cases, because of "pain" or "moderate pain" and remains in the Department for 127 minutes. This pacient is older, receives "very urgent " or "emergent " triage more frequently than other users and spends substantially more time in the Emergency Room than other users.
