Apesar de tanto a trombólise como a trombectomia mecânica se terem provado formas
de tratamento eficazes para doentes com AVC isquémico, o acesso a estas terapêuticas é
dificultado pelos restritos limites temporais para administração de tratamento impostos
pelas guidelines. Estudos anteriores mostraram que a partir de algumas horas após o
início dos sintomas (6h para a trombectomia e 3h para a trombólise) a relação riscobenefício deixava de estar em favor da administração de tratamento. No entanto,
trabalhos mais recentes indicam que através de exames de neuroimagem,
nomeadamente da Ressonância Magnética e da TC de perfusão, é possível identificar
doentes que, independentemente do tempo de evolução dos seus sintomas, possam ainda
beneficiar dos efeitos do tratamento.
Although both thrombolysis and mechanical thrombectomy have been proven effective forms of treatment for patients with ischemic stroke, access to these treatments is hampered by strict time limits for treatment administration imposed by the guidelines. Previous studies have observed that after a few hours after symptom onset (6h for thrombectomy and 3h for thrombolysis) the risk-benefit ratio is no longer in favor of administering treatment. However, recent works show that through neuroimaging exams, namely Magnetic Resonance and perfusion CT, it is possible to identify patients who, regardless of the time of evolution of their symptoms, may still benefit from the effects of the treatment.
