dc.description.abstract	A hipertensão pulmonar persistente do recém-nascido (HPPRN) é um síndrome clínico complexo com múltiplas causas que resulta da incapacidade da circulação pulmonar fetal fazer a transição para a vida extra-uterina. Define-se como uma resistência vascular pulmonar aumentada e shunt direito-esquerdo através do foramen ovale e/ou do ductus arteriosus, causando hipoxémia arterial refractária à suplementação de oxigénio. Com o aparecimento de novas modalidades terapêuticas, fruto de marcados avanços ocorridos nos últimos anos, o prognóstico desta patologia melhorou significativamente. Não obstante, a HPPRN continua a representar um desafio terapêutico. A recolha do material bibliográfico utilizado na elaboração da presente tese foi realizada através de uma pesquisa na Medline, usando a interface de pesquisa Pubmed com as palavras-chave “persistent pulmonary hypertension newborn” pesquisadas no título e abstract. A pesquisa foi limitada aos artigos publicados entre 01/01/2003 e 2008, escritos em Inglês, Francês, Espanhol e Português. Dos resultados obtidos foram seleccionados os artigos mais genéricos sobre etiologia, fisiopatologia, manifestações clínicas e diagnóstico e todos aqueles relacionados com o tratamento. Utilizaram-se também os Consensos Nacionais de Neonatologia 2004 da Sociedade Portuguesa de Pediatria, os Protocolos da Asociación Española de Pediatría de 2003 e livros disponíveis sobre o tema. A presente monografia faz uma revisão do actual estado da arte no que diz respeito à terapêutica da HPPRN, nomeadamente suporte ventilatório, vasodilatadores pulmonares, surfactante e oxigenação por membrana extracorporal.
dc.description.abstract	The persistent pulmonary hypertension of the newborn (PPHN) is a complex clinical syndrome with multiple causes that result from the failure of the fetal pulmonary circulation transition for the extrauterine life. It is defined as an increased pulmonary vascular resistance and righ-to-left shunt across the foramen ovale and/or the ductus arteriosus, causing arterial hypoxemia refractary to the oxygen supplementation. With the advent of new management strategies, resulting from marked advances occurred in recent years, the prognostic of this pathology improved significantly. However, the PPHN continues to represent a therapeutical challenge. The gathering of the bibliographical material used in the elaboration of this thesis was carried through a research in Medline, using the research interface Pubmed with the keywords “persistent pulmonary hypertension newborn” searched in the title and abstract. The research was limited to articles published between 01/01/2003 and 2008, writen in English, French, Spanish and Portuguese. From the results the most generic articles about etiology, pathophysiology, clinical manifestations and diagnostic and all those related with the treatment were selected. The Consensos Nacionais de Neonatologia 2004 from Sociedade Portuguesa de Pediatria, the Protocols of the Asociación Española de Pediatría from 2003 and available books on the subject were also used. The present monograph makes a revision of the current state of the art in what it concerns the therapeutic of PPHN, such as ventilatory support, pulmonary vasodilators, surfactant and extracorporeal membrane oxygenation.
