Abstract(s)
A presente dissertação encontra-se dividida em três capítulos. O primeiro capítulo diz
respeito à investigação desenvolvida no âmbito do estudo sobre normas para a preparação e
administração de citotóxicos injetáveis. O objetivo deste estudo foi a realização de um guia
sobre reconstituição e/ou diluição, conservação e estabilidade após reconstituição e/ou
diluição, vias de administração autorizadas, velocidade de perfusão, para além de outras
observações consideradas pertinentes. Adicionalmente, realizou-se um inquérito aos hospitais
portugueses que preparam quimioterapia injetável, de forma a avaliar procedimentos de
preparação de citotóxicos injetáveis e compreender qual o interesse na publicação do guia.
A preparação de citotóxicos administrados por via parentérica constitui uma atividade chave
de muitos serviços farmacêuticos hospitalares. Devido à crescente disponibilidade de
medicamentos citotóxicos, quer de marca, quer genéricos, tem aumentado o tempo
despendido pelos farmacêuticos hospitalares na pesquisa de informação relativa a
reconstituição e/ou diluição, conservação e estabilidade destes medicamentos. De forma a
dar uma resposta rápida e eficaz no que respeita à obtenção daquela informação é
conveniente dispor de uma base de dados que reúna toda aquela informação para todos os
medicamentos citotóxicos disponíveis em Portugal.
Os resultados obtidos do inquérito aplicado aos hospitais portugueses que preparam
quimioterapia injetável, revelam que a maioria dos procedimentos de preparação são
realizados por farmacêuticos ou com a sua supervisão (65%). Todos os hospitais incluídos no
estudo preparam os citotóxicos em câmaras de fluxo de ar laminar vertical e 70% deles
afirmam conter as câmaras em sistemas modulares de salas limpas. Quanto ao interesse na
realização e disponibilização de um guia sobre a reconstituição, diluição, administração e
conservação de citotóxicos injetáveis, a resposta dos hospitais foi unânime, concluindo-se que
a divulgação do livro é de extrema importância para os profissionais de saúde.
O segundo capítulo aborda as competências adquiridas e as atividades que acompanhei
durante o estágio realizado em farmácia hospitalar e o terceiro capítulo durante o estágio
realizado em farmácia comunitária.
The present dissertation is divided intro three chapters. The first one concerns on the research undertaken as part of the study on standards for the preparation and administration of injectable cytotoxics. The aim of this study was to carry out a guide on reconstitution and/or dilution, conservation and stability after reconstitution and/or dilution, routes of administration, authorized infusion rate, in addition to other observations considered relevant. Additionally, it was carried out a survey of Portuguese hospitals that prepare injectable chemotherapy, in order to evaluate procedures of preparation of injectable cytotoxics and to understand the interest of the publication of the guide. The preparation of injectable cytotoxics is a key activity of many hospital pharmaceutical services. Due to the increasing availability of cytotoxic medicines, either branded or generic, the time spent by hospital pharmacists in search of information about reconstitution and/or dilution, conservation and stability of these drugs has increased. In order to effectively respond to this information need, it would be useful to have a database that gathers all that information for all cytotoxic medicines currently available in Portugal. The results of the survey applied to Portuguese hospitals that prepare injectable chemotherapy, reveals that most of the preparation procedures are performed by pharmacists or with their supervision (65%). All hospitals included on the study prepare cytotoxics in biological safety cabinets and 70% of them claim to have these cameras inserted in modular clean rooms. On the interest in the performance and availability of a guide on reconstitution, dilution, administration and conservation of injectable cytotoxics, hospitals’ response was unanimous, concluding that publication of the book is extremely important for healthcare professionals. The second chapter discusses the acquired skills and activities developed and followed during the training held in hospital pharmacy, and the third one during the training held in community pharmacy.
