Abstract(s)
Introdução: A perda auditiva corresponde a uma das principais causas de incapacidade
a nível mundial. Neste contexto, o implante coclear, que constitui um dos avanços mais
impressionantes da medicina moderna, assume um papel preponderante não só
terapêutico, como também a nível linguístico, cognitivo, emocional e de desenvolvimento
social, contribuindo para a melhoria da qualidade de vida dos indivíduos. Inicialmente,
a colocação de implantes cocleares restringia-se a doentes com hipoacúsia
neurossensorial profunda bilateral. No entanto, as indicações têm vindo a expandir
gradualmente ao longo dos últimos anos.
Objetivos: Avaliar quais as indicações atuais para a colocação de implantes cocleares
em adultos.
Métodos: A pesquisa, análise e seleção de bibliografia foram realizadas durante os
meses de setembro a novembro de 2023. A pesquisa foi realizada na base de dados
PubMed/MEDLINE e foram selecionados artigos entre janeiro de 2018 e novembro de
2023. Utilizou-se a seguinte sequência de caracteres: (“cochlear implant” OR “cochlear
implantation”) AND (“indications” OR “criteria”). Foram incluídos artigos que
reportassem informação científica sobre indicações atuais para a colocação de implante
coclear standard, realizados em seres humanos, com idade superior ou igual a 18 anos e,
por fim, escritos em inglês e português.
Resultados: Dos 26 artigos científicos incluídos, foram estudadas as indicações para a
colocação de implante coclear em adultos atualmente definidas para 22 países. De uma
forma geral, nenhum país estabelece um limite etário máximo para a colocação de
implante coclear e verifica-se que tanto a implantação bilateral, como a implantação em
casos de surdez unilateral e assimétrica, têm sido cada vez mais frequentes entre os
vários países.
Conclusão: É possível constatar uma expansão nas indicações para a colocação de
implante coclear em adultos, embora com discrepâncias entre os países e variabilidade a
nível nacional em alguns deles. Alguns países acabam por ser mais permissivos do que
outros, o que contribui para a heterogeneidade dos critérios que, em Portugal,
atualmente, ainda não foram estabelecidos. Apesar da crescente expansão referida,
devido a constrangimentos inerentes e extrínsecos aos utentes, continua a verificar-se
uma subutilização destes dispositivos.

In this context, the cochlear implant, which represents one of the most impressive advancements in modern medicine, plays a preeminent role not only therapeutically but also linguistically, cognitively, emotionally and in social development, contributing to the improvement of individuals' quality of life. Initially, cochlear implantation was restricted to patients with bilateral profound sensorineural hearing loss. However, the indications have been gradually expanding over the past few years. Objective: Assessing the current indications for cochlear implant placement in adults. Methods: The research, analysis and literature selection were conducted from September to November 2023. The search was performed in the PubMed/MEDLINE database and articles between January 2018 and November 2023 were selected. The following search string was used: (“cochlear implant” OR “cochlear implantation”) AND (“indications” OR “criteria”). Articles reporting scientific information on current indications for standard cochlear implantation in humans, aged 18 years and older were included and we selected only the ones written in English and Portuguese. Results: Of the 26 scientific articles included, we found indications for cochlear implantation in adults currently defined for 22 countries. Overall, no country establishes a maximum age limit for cochlear implantation placement and it is observed that both bilateral implantation and implantation in cases of unilateral and asymmetric deafness have been increasing among various countries. Conclusion: We can observe an expansion in the indications for cochlear implant placement in adults, although with discrepancies between countries and variability at national level in some of them. Some countries end up being more permissive than others, contributing to the heterogeneity of criteria that, in Portugal, have not yet been established. Despite the mentioned growing expansion, due to inherent and extrinsic constraints to users, there continues to be underutilization of these devices.
