A ecografia Point-of-Care (POCUS) é uma ferramenta inovadora e valiosa no contexto da
emergência médica pré-hospitalar, permitindo diagnósticos mais rápidos e precisos,
além de reduzir a necessidade de procedimentos invasivos. Com esta dissertação
procurou-se investigar o conhecimento e as perspetivas dos médicos e enfermeiros da
emergência pré-hospitalar em Portugal sobre a POCUS e analisar as vantagens
percebidas e as barreiras à sua implementação na emergência pré-hospitalar.
Para isso foi aplicado um questionário a médicos e enfermeiros a médicos e enfermeiros
que exercem na emergência pré-hospitalar a nível nacional. Os resultados obtidos,
através das 110 respostas, demostraram que 95,5% destes profissionais reconhecem os
benefícios da POCUS, sendo que 60% dos inquiridos referem possuir conhecimentos em
POCUS, mas apenas 4,5% a utilizam em contexto pré-hospitalar.
Em relação às principais barreiras, foram identificadas a falta de equipamentos e de
formação específica, sendo que a maioria adquiriu formação por iniciativa própria,
através de cursos pagos pessoalmente ou por aprendizagem autodidata. Além disso,
32,4% dos profissionais apontaram a falta de treino como uma dificuldade e 18,1%
mencionaram não possuir conhecimento suficiente para a sua aplicação. Entre os
restantes fatores, alguns referiram receio de que o uso da POCUS possa atrasar a
abordagem ao paciente, enquanto uma minoria (3,8%) considerou que esta tecnologia
não teria utilidade na prática pré-hospitalar.
Destacou-se ainda que os médicos demonstraram maior familiaridade na utilização da
POCUS comparativamente aos enfermeiros.
No âmbito de perspetivas futuras, seria relevante realizar estudos adicionais para avaliar
o custo-benefício da implementação da POCUS na emergência pré-hospitalar
portuguesa, analisando os seus impactos na redução da morbimortalidade e na eficiência
dos serviços de emergência
Assim, este estudo reforça a necessidade de aumentar a disponibilidade de dispositivos
de ecografia portátil nos serviços de emergência médica em Portugal, bem como de
investir em programas de formação estruturados, acessíveis e direcionados para os
profissionais do pré-hospitalar.
Point-of-Care Ultrasound (POCUS) is an innovative and valuable tool in the context of prehospital emergency medicine, enabling faster and more accurate diagnoses while reducing the need for invasive procedures. This dissertation aimed to investigate the knowledge and perspectives of prehospital emergency physicians and nurses in Portugal regarding POCUS and to analyze the perceived advantages and barriers to its implementation in prehospital emergency care. To achieve this, a questionnaire was administered to physicians and nurses working in prehospital emergency services nationwide. The results, based on 110 responses, showed that 95.5% of these professionals recognize the benefits of POCUS, while 60% reported having some knowledge of POCUS, but only 4.5% use it in prehospital settings. Regarding the main barriers, the lack of equipment and specific training were identified as key obstacles. Most respondents acquired training through personal initiative, either by attending self-funded courses or through self-directed learning. Additionally, 32.4% of professionals pointed to a lack of training as a challenge, while 18.1% stated they did not have sufficient knowledge to apply POCUS. Among other factors, some expressed concerns that using POCUS might delay patient management, while a minority (3.8%) considered this technology to be of no practical use in prehospital care. It was also noted that physicians demonstrated greater familiarity with POCUS compared to nurses. For future perspectives, further studies would be relevant to assess the cost-benefit of implementing POCUS in Portuguese prehospital emergency care, analyzing its impact on reducing morbidity and mortality and improving the efficiency of emergency services. Thus, this study highlights the need to increase the availability of portable ultrasound devices in emergency medical services in Portugal, as well as to invest in structured, accessible training programs tailored to prehospital professionals.
