Abstract(s)
O presente relatório, intitulado “Suspeita de Glaucoma, Adaptação de lentes de contacto
multifocais e Pseudo-insuficiência de convergência”, é constituído por três casos clínicos,
da área dos cuidados primários da visão, que foram selecionados no decorrer do estágio
realizado no Centro Clínico e Experimental em Ciências da Visão (CCECV) na
Universidade da Beira Interior (UBI).
O primeiro caso aborda uma suspeita de Glaucoma com pressão intraocular elevada.
Caracterizado por alterações ao nível do disco ótico e do campo visual, este caso permitiu
expor a importância do optometrista não só durante o processo de triagem em situações
patológicas, como na realização e interpretação de exames complementares de
diagnóstico na prática clínica de modo a identificar sinais suspeitos de patologia e sua
referenciação.
O segundo caso diz respeito a uma adaptação de Lentes de contacto hidrófilas multifocais
tóricas. Na prática clínica é cada vez mais recorrente, em pacientes presbitas, a adaptação
de LC multifocais de modo a eliminarem a necessidade de utilização de óculos em visão
próxima, quando utilizam lentes de contacto monofocais. Enfatizou-se a problemática
das geometrias das lentes multifocais, com especial atenção às geometrias assimétricas
em casos de adições baixas.
Por último, o terceiro caso refere-se a uma anomalia da visão binocular, pseudoinsuficiência de convergência. É uma condição que apresenta características tanto de
insuficiência de convergência como de insuficiência acomodativa.
Esta disfunção ocorre em casos em que o principal problema é a insuficiência
acomodativa que levou, posteriormente, ao aparecimento de uma insuficiência de
convergência. Para a resolução deste caso foi proposto ao paciente um plano de terapia
visual como solução para a sintomatologia apresentada.
The present work consists of three case reports, which is titled “Suspicious Glaucoma, Multifocal contact lenses fitting and convergence pseudo-insufficiency”. These cases are included in primary vision care field and were selected during the internship at the Clinical and Experimental Centre in Optometry and Vision Sciences at the University of Beira Interior. The first case approaches a suspicious glaucoma with high intraocular pressure. Characterized by changes at the level of the optic disk and the visual fields, this case allowed to expose the optometrist’s importance during the screening process in pathological situations, but also in the performance and interpretation of the complementary diagnostic exams in the clinical practice, to report and identify suspicious pathological signals. The second case approaches a multifocal toric contact lens fitting. In the clinical practice is recurrent, in presbyopic patients, the use of this type of contact lens to avoid the need to use near vision spectacles, when wearing monofocal contact lens. In this clinical case, the problem with contact lens geometry was emphasized, with special attention to asymmetric geometries in low additions cases. The third case refer a binocular vision dysfunction, convergence pseudo-insufficiency. It is a condition that presents convergence insufficiency as accommodative insufficiency characteristics. This dysfunction occurs in cases that the primary problem is the accommodative insufficiency that subsequently led to a convergence insufficiency. For the management of this case, it was proposed a visual therapy plan as a solution to the symptomatology reported.
