Este relatório de estágio encontra-se dividido em três capítulos. Cada capítulo corresponde à experiência profissionalizante adquirida nas diferentes componentes do estágio. Os capítulos respeitantes à farmácia comunitária e à farmácia hospitalar foram estruturados de forma a descrever resumidamente a experiência vivida.
O primeiro capítulo do relatório, relacionado com a vertente de investigação, pretende descrever o projeto de dissertação de mestrado na área da farmácia hospitalar intitulado “Seguimento Farmacoterapêutico num Grupo de Doentes Crónicos”. O principal objetivo deste trabalho foi avaliar e estudar a adesão à terapêutica por parte dos doentes utilizando dois tipos de métodos indiretos. A adesão à terapêutica tem vindo a ser uma preocupação crescente dos profissionais de saúde, pois é um importante modificador da efetividade dos sistemas de saúde. As consequências da má adesão repercutem-se em resultados desfavoráveis nos indicadores de saúde e no aumento das despesas em saúde. As causas da não adesão são múltiplas, podendo depender do doente, do medicamento e de condições económicas. Existe um conjunto de intervenções eficazes na melhoria da adesão, mas ainda não se pode afirmar com segurança quais as mais eficazes. Neste sentido o presente trabalho descreve o acompanhamento de um número restrito de doentes em consulta farmacêutica em ambiente privado e confidencial.
O segundo capítulo pretende descrever todas as competências técnico-científicas adquiridas, atividades desenvolvidas e metodologias observadas ao longo do estágio em farmácia hospitalar. O estágio realizou-se nos serviços farmacêuticos do Centro Hospitalar Cova da Beira durante o período de 18 de junho a 2 de agosto de 2012.
O terceiro capítulo do relatório tem o propósito de descrever o funcionamento de uma farmácia comunitária e as múltiplas tarefas e responsabilidades que são da competência do farmacêutico neste âmbito profissional. Este estágio decorreu na Farmácia Moderna de Esmoriz no período de 17 de setembro a 15 de dezembro.
This supervised academic report is divided in three chapters. Each chapter corresponds to the vocational experience acquired in the different components of the training. The chapters respective to community pharmacy and hospital pharmacy were structured in order to summarize the lived experience in each phase of the training. The first chapter of the report related with the investigation under discussion wants to describe the dissertation project about hospital pharmacy called “Pharmacotherapeutic Follow-up in a Group of Chronic Patients”. The main goal of this work was to assess and study the therapeutic adherence by the patients utilizing two types of indirect methods. The therapeutic adherence has been a growing concern for the health professionals for it is an important effectiveness modifier of the healthcare systems. The consequences of poor adherence reverberate in unfavourable results in the healthcare indicators and in increasing health expenses. There are multiple causes for nonadherence, they might depend from the patient, the medicine or from economic conditions. There is a group of effective interventions in the improvement of adherence, but it’s still too soon to tell which ones are more effective. This dissertation describes the follow-up of a restricted number of patients in pharmaceutical appointment/consultation in a private and confidential environment. The second chapter wants to describe all acquired skills both technical and scientific, developed activities and methodologies observed during the training in hospital pharmacy. The training took place in the pharmaceutical services of Centro Hospitalar Cova da Beira during the period of July, 18th to August, 2nd of 2012. The third chapter of the report has the purpose of describing the working of a community pharmacy and its multiple tasks and responsibilities that are the skills of the pharmacist in his daily work. This training took place in Farmácia Moderna de Esmoriz during the period between September, 17th to December, 15th of 2012.
