O transplante renal é hoje considerado como a melhor arma terapêutica para os doentes
renais em estadio final, aumentando não só a sobrevida dos doentes, como também a sua
qualidade de vida, relativamente aos doentes em diálise.
A rejeição tem sido um dos obstáculos à sua plena implementação. Recorre-se a testes de
histocompatibilidade e a potentes fármacos imunossupressores para controlar a rejeição e
promover a tolerância.
Nesta monografia abordamos os mecanismos celulares e moleculares presentes rejeição
(celular e humoral) e na tolerância.
Também fazemos referência às estratégias indutoras de tolerância, assim como aos
principais fármacos incluídos nos protocolos imunossupressores.
Renal transplantation is presently seen as the best choice for treatment of the end stage renal disease, improving patient survival and quality of life when compared with patients on dialysis. Rejection has been a major obstacle to its widespread implementation. Histocompatibility tests and powerful immunosuppressive drugs are used to control rejection, and promote tolerance. In this monograph we describe the molecular and cellular mechanisms of rejection (cellular and humoral) and tolerance. We also discuss tolerance-induction strategies, as well as to the drugs included in the immunosuppressive protocols.
