Exposições nutricionais precoces são determinantes significativos no desenvolvimento e saúde futura de todos os sistemas de órgãos. O aumento dramático das doenças imunes infantis, mais notavelmente da doença alérgica, indica a vulnerabilidade específica do sistema imunitário às alterações ambientais precoces.
Compreender como as influências ambientais actuam no desenvolvimento imunológico e metabólico é de fundamental importância. O leite materno contém muitos factores imunológicos que compensam os mecanismos de defesa subdesenvolvidos no recém-nascido, no entanto, por vezes, pode ser necessária a introdução precoce dos alimentos sólidos.
Vários estudos epidemiológicos relacionaram um menor risco de aparecimento de doença alérgica com a introdução precoce da proteína do leite de vaca na alimentação da criança. Uma comparação e melhor compreensão da composição dos leites materno e de vaca, realçando os componentes com possível função imunológica, bem como o seu mecanismo de acção, é fundamental para perceber em que medida o seu consumo vai ter repercussões no aparecimento da doença alérgica.
São apresentados os principais dados relativos à idade de início da diversificação, e ao modo de progressão da alimentação ao longo do primeiro ano de vida, bem como da importância do aleitamento materno exclusivo, de acordo com as recomendações quer Nacionais, quer Europeias.
Por último, é mostrado de que forma a introdução precoce dos alimentos influencia a doença alérgica respiratória (asma e rinite alérgica) e tópica (eczema/dermatite atópica).
Early nutritional exposures are significant determinants in the development and future health of all organ systems. The dramatic increase of infant immune diseases, most notably allergic disease, indicates the particular vulnerability of the immune system to early environmental changes. Breast milk contains many immune factors that compensate the underdeveloped defense mechanisms in the newborn, however, sometimes the early introduction of solid foods may be required. Several epidemiological studies have shown a lower risk of development of allergic disease with the early introduction of cow's milk protein in infant feeding. The comparison and better understanding of the composition of breast and cow milk, enhancing ingredients with possible immune function as well as their mechanism of action is fundamental to realize that measuring the consumption will have implications on allergic diseases occurrence. There are submitted the key data, the age of onset of diversification, and the method of feeding along the first year of life, as well as the importance of exclusive breastfeeding according with both National and European recommendations. Lastly it is shown how the early introduction of food influences allergic (asthma and allergic rhinitis) as well as topical (eczema/atopic dermatitis) respiratory diseases.
