O presente relatório de estágio relata a experiência adquirida aquando do estágio curricular no Centro Clínico e Experimental em Ciência da Visão, uma unidade da Faculdade de Ciências da Saúde da Universidade da Beira Interior, que promove os cuidados primários em saúde visual junto da comunidade, em diversas áreas específicas da Optometria. A participação nos rastreios da retinopatia diabética no UBIMedical e no Projeto V+, e os equipamentos disponíveis, permitiram adquirir conhecimentos e metodologias de trabalho, que não seriam possíveis adquirir em óticas comerciais.
Neste relatório são descritos três casos clínicos: um caso clínico de patologia ocular, um caso clínico relativo a alterações na visão binocular e por último um caso clínico referente à adaptação de lentes de contato.
O caso clínico de patologia ocular, refere-se a uma paciente com sinais de neuropatia ótica normativa. Verificadas perdas de campo visual na Perimetria Estática Computorizada, o caso foi referenciado para a especialidade de oftalmologia. Este caso revela a importância de uma observação sistemática e cuidadosa do fundo ocular e dos meios complementares de diagnóstico.
O segundo caso clínico, refere-se a uma estudante inserida no Projeto V+, que após o rastreio nas escolas foi referenciada para o Centro Clínico e Experimental em Ciências da Visão para uma consulta de optometria. Realizados os exames optométricos necessários, diagnosticou-se uma Pseudo-insuficiência de convergência. Para correção deste problema de visão binocular recorreu-se ao treino visual. Foi essencial para este caso a criação de uma relação paciente-optometrista e constante motivação do paciente.
O terceiro caso referido neste relatório descreve a adaptação de lentes de contacto hidrófilas tóricas numa paciente de 64 anos com uma miopia elevada. Descartada a possibilidade de adaptação de lentes de contacto rígidas permeáveis aos gases, foram expostas todas as opções de adaptação com lentes hidrófilas. Após adaptadas lentes de contacto hidrófilas tóricas, a pacientes não se adaptou à perda de visão ao perto, sendo então sugerido e adaptado lentes de contacto hidrófilas multifocais. Este caso mostra a relevância da anamnese, da motivação e expectativas do paciente na adaptação de lentes de contacto.
The present stage report reports on the experience gained during the internship at Centro Clínico e Experimental em Ciência da Visão, a unit of the Faculty of Health Sciences of the Universidade da Beira Interior, which promotes primary visual health care in the community, in several specific areas of optometry. Participation in the screening of diabetic retinopathy in UBIMedical and in the V+ Project, and the avaiable equipment, allow to acquire knowledge and work methodologies that would not be possible to acquire in optical stores. In this report are described three Clinical Cases: a case of ocular pathology, a case concerning the deviations in binocular vision and finally a case related to a contact lens adaptation. The clinical case of ocular pathology, refers to a patient with signs of optic neuropathy. Verified Visual field defects in the Computerized Static Perimetry, the case was referenced the Ophthalmology specialty. This case reveals the importance of a systematic and careful observation of the ocular fundus and the complementary means of diagnosis and therapy. The second clinical case, refers to a student that entered in the V+ Project, which after the screening in the schools, was referenced to the Centro Clínico e Experimental em Ciências da Visão to an optometric examination. After conducting the necessary optometric exams, Pseudo-convergence insufficiency was diagnosed. To correct this binocular vision problem vision therapy was used. It was essential for this case to create a patient-optometrist relationship and constant patient motivation. The third case referred in this report describes the adaptation of soft toric contact lenses in a 64-year-old patient with high myopia. Ruled out the possibility of adapting rigid gas permeable contact lenses, all option for soft lenses adaptation were exposed. After soft toric contact lenses were adapted, the patient didn’t adapt to the loss of near vision, being then suggested and adapted soft multifocal contact lenses. This case shows the relevance of anamnesis, motivation and expectations of the patient in the adaptation of contact lenses.
