Abstract(s)
Em Portugal, a empresarialização da gestão das instituições de saúde iniciou-se na
última década do século XX, passando por parcerias público-privadas, sociedades
anónimas e entidades públicas empresariais (EPE), culminando com a generalização
desta última modalidade, através da integração de todas as instituições públicas de saúde
(à exceção dos 3 institutos de oncologia) em Unidades Locais de Saúde (ULS), EPE, a
partir de 01 de janeiro de 2024. O processo de contratualização em saúde apresenta-se
como resposta às exigências de uma gestão por objetivos, que se quer proativa. De acordo
com Antunes, et al. (2011), no processo de contratualização em saúde, a relação entre
financiador e prestadores, aproxima-se das noções da Nova Gestão Pública, como sejam,
as relações contratuais, a definição clara de papéis, a responsabilização das partes
envolvidas e a maximização dos objetivos de eficiência, eficácia e economia.
No contexto da gestão dos serviços de saúde, a adoção de ferramentas para o cálculo e a
análise de índices de desempenho é essencial para assegurar decisões informadas e
promover a melhoria contínua da qualidade dos serviços prestados. Nesse sentido, surge
a seguinte questão de investigação: de que forma a criação de um simulador de índices
de desempenho em Excel pode influenciar e melhorar a eficácia do processo de
contratualização dos CSP na ULS Guarda, tanto em termos de precisão na tomada de
decisão baseada em dados quantitativos, como na melhoria global dos indicadores de
desempenho dos serviços de saúde?
O presente projeto de mestrado responde à questão da investigação com uma revisão
teórica sobre temas que suportam o desenvolvimento e implementação de um simulador
em Excel em contexto de contratualização. O simulador foi implementado e validado em
contexto real, sendo avaliado como uma ferramenta de fácil utilização, intuitiva e bem
organizada, que contribui para uma negociação mais eficaz e informada, com
funcionalidades que atendem às necessidades das unidades funcionais.
In Portugal, the corporatization of the management of health institutions began in the last decade of the 20th century, involving public-private partnerships, joint-stock companies, and public business entities. This process culminated in the generalization of the public business entities through the integration of all public health institutions (except for the three oncology institutes) into Local Health Units, starting on January 1, 2024. The process of contractualization in healthcare emerges as a response to the demands of goal-oriented management, which aims to be proactive. According to Antunes, et al. (2011), in the healthcare contractualization process, the relationship between the funder and providers approaches the notions of New Public Management, such as contractual relationships, clear definition of roles, accountability of the parties, and the maximization of efficiency, effectiveness, and economy objectives. In the context of health services management, the adoption of tools for the calculation and analysis of performance indexes is essential to ensure informed decisions and promote the continuous improvement of the quality of services provided. In this regard, the following research question arises: how can the development of a performance indexes simulator in Excel influence and improve the effectiveness of the contractualization process of Primary Health Care in Local Health Unit of Guarda, both in terms of accuracy in decision-making based on quantitative data and in the overall improvement of health service performance indicators? This master's project answers the research question with a theoretical review of topics that support the development and implementation of an Excel simulator in the context of contractualization. The simulator was validated in a real-life context and was evaluated by the participants as an easy-to-use, intuitive and well-organized tool that contributed to more effective and informed negotiations, with features that met the needs of the functional units.
