Abstract(s)
O principal objectivo do estudo centrou-se na avaliação da capacidade das faculdades de
medicina nacionais durante o 6º ano profissionalizante na promoção das competências teórico-práticos indispensáveis à prática clínica inicial do médico interno.
O questionário aplicado identificou alguns aspectos comuns a todas as faculdades de
medicina nacionais incluídas no estudo. Contudo, as diferenças entre faculdades não foram
valorizadas tendo em conta o número reduzido de elementos da amostra, a fraca adesão ao
preenchimento do questionário apresentado e a ausência de representatividade de três das
faculdades de medicina nacionais. A sua aplicação no estudo não foi realizada com o intuito
de retirar inferência ou significância estatística, mas antes com a pretensão de avaliar
qualitativamente os dados colhidos numa base de recolha de opinião.
Alguns pontos-chave foram identificados: a formação teórica como ponte de ligação para a
prática clínica é excelente; as competências teórico-práticas que a grande maioria dos
médicos internos está apta a efectuar no início das suas carreiras inclui a colheita da história
clínica, a realização de exame objectivo e a procura e colheita da informação necessária à sua
autoaprendizagem; em relação às competências exclusivamente práticas a grande maioria dos
internos apenas se mostra apta a efectuar punção arterial, punção venosa, otoscopia,
especuloscopia, suturas e SBV; a farmacologia/prescrição terapêutica e a necessidade de
maior prática clínica e menos teoria são apontadas como as grandes falhas dos planos
curriculares do actual 6º ano profissionalizante; a adequada formação em prescrição terapêutica, as competências práticas bem definidas, o priviligear o lema “aprendendo,
fazendo” e a habilitação à prestação de cuidados em serviços de urgência e emergência foram
apontados como os aspectos fundamentais a integrar no 6º ano profissionalizante caso o ano
comum venha a ser ou não excluído das carreiras médicas; por último, a grande maioria dos
médicos internos gostaria de ver incorporada na futura prática clínica uma actualização
contínua dos seus conhecimentos médico-científicos, uma visão holística do paciente, a
valorização pelo trabalho em equipa e a capacidade de assumir a responsabilidade pelos actos
praticados.
Em conclusão, a grande maioria dos médicos internos do estudo nega ter recebido uma
adequada preparação académica para assumir o papel de interno, reconhecendo a necessidade
de maior prática clínica durante o 6º ano profissionalizante.
The main purpose of the study was to evaluate the medical school training and education during the 6th year programme on the preparation of medical students for being an intern. The questionnaire has identified some key trends common to all medical schools. However, differences between faculties were not valued regarding the small sample’s dimension, the poor adherence to complete the questionnaire and the absence of three of all portuguese medical schools. Its application in the study was not drawn to take any inference or statistical significance, but rather with the intention of reporting general opinions and assessing collected data in a qualitative way. Some of the key issues identified include: intern’s theoretical preparation is excellent and provides a sound basis for the development of clinical skills; final 6th year students have good preparation in history taking, carrying out objective clinical examinations and knowing where to find information; they also have good preparation for sampling arterial and venous blood, otoscopy, gynaecological examinations, suturing and performing basic cardiopulmonary resuscitation; topics which are deemed of most use for the internship but which are not well covered in the undergraduate programme include pharmacology/therapeutics and the need for more clinical practice and less theory during the 6th year; including sufficient and appropriate opportunities for students to be engaged in clinical care, to practise their clinical skills and to have emergency room experience are the aspects of the intern period that should be included in the undergraduate course if there was or wasn’t no internship; by last, the vast majority of interns would like to take continuous updating of their medical and scientific knowledge, to take an holistic view of one´s patients, expect to learn a lot from other health professionals through team work and taking responsibility for their own practice and skills. In conclusion, the vast majority of interns felt that they weren’t well prepared for the internship period recognizing the need of much more clinical practice during the 6th year programmes.
